Not medical or legal advice
Nothing on this website is medical, legal, or benefits advice. Eligibility for services is determined by the Local Intellectual and Developmental Disability Authority (LIDDA) and Medicaid approval, and the scope and cost of services are determined by each person's Individual Plan of Care (IPC) and Medicaid waiver authorization. Submitting a referral or inquiry does not create a service relationship.
Use of the site
You agree to use this website lawfully and to provide accurate information in our forms. Please do not submit information about another person unless you are authorized to do so, for example as a guardian, family member, or professional referral source.
